Summary: For students ages 14-21 years old with developmental disabilities, transitioning is the process students and their families use to think about life after high school. The purpose of this process is to help students move successfully from school to post-secondary education, training, employment, or independent living. Community participation is based on the student's preferences, interests, and abilities.
