The Pennsylvania Health Law Project, thanks to funding from the Pennsylvania Developmental Disabilities Council, has created a “Community HealthChoices 101” Fact Sheet to provide people with disabilities, their families and other stakeholders with basic information on the program.
Pennsylvania’s Community Health Choices (CHC) implementation will roll out in three phases over two years beginning January 1, 2018.
